Application for Leadership Training
This should be an individual. Every applicant should have a person who provides direction, focus, and accountability before and after the leadership training. Examples of sponsors include your employer or leader including labor leader, spiritual leader, or officer of civic, political, faith or labor organization.
If different from "Your Organization Affiliation" listed above. This should be the organization that is sending you to training and investing in your leadership.
What do you hope to achieve and/or do differently as a result of being in the training?
Examples: Executive Committee, Board member, Staff Organizer, Fundraiser, Elected office
 
On a scale of 1 to 10 (with 10 being the top), to what extent are you the kind of leader you want to be? Describe what your number means in a few sentences.
What makes this training timely for your organization? Example: organizational development issues, leadership turnover, policy challenges, demographic changes, etc.
List any significant leadership training that you have had in the past indicating the topic/subject, who trained you, whether it was sponsored by your work or by an organization you are (or were) associated with.
